HopeHealth has received two Community Health Quality Recognition (CHQR) awards from the Health Resources & Services Administration (HRSA). The organization was recognized with the gold Health Center Quality Leader badge for the second consecutive year and also earned the National Quality Leader – Heart Health badge.
The gold status is awarded to health centers that achieve the highest overall clinical quality measure performance among all centers. This distinction places HopeHealth in the top 10 percent of centers nationwide for overall quality performance. Silver and bronze badges are given to those in the next highest percentiles.
To receive the National Quality Leader – Heart Health award, HopeHealth met or exceeded three out of four national targets for cardiovascular care. These targets include tobacco use screening and cessation intervention, use of aspirin or another antiplatelet for ischemic vascular disease, statin therapy for prevention and treatment of cardiovascular disease, and hypertension control.
Community health centers like HopeHealth report data each year on patient characteristics, services provided, clinical processes and outcomes, service usage, staffing, costs, and revenues through a standardized system called Uniform Data Systems (UDS).
